Mangelfullt krisesentertilbud til voldsutsatte med alvorlig problematikk knyttet til rus, psykiske lidelser og funkjsonsnedsettelse

Det er overhyppighet av voldsforekomst blant voldsutsatte med alvorlig problematikk knyttet til rus, psykiske lidelser og funksjonsnedsettelse sammenlignet med den generelle befolkningen. Majoriteten av kommunene leverer ikke et tilfredsstillende krisesentertilbud til disse gruppene. Dagens tjenestetilbud er svært variert, og ulike kommuner har ulike forutsetninger for å tilrettelegge for disse spesielt sårbare gruppene.

På oppdrag fra Bufdir har Varde Hartmark utarbeidet tre viktige rapporter etter utviklingsprosjekt for å styrke krisesentertilbudet i kommunene til voldsutsatte med alvorlig problematikk knyttet til rus, psykiske lidelser og funksjonsnedsettelse.

Krisesenteret i Salten, Krisesenteret i Molde og Omegn og Krisesenteret i Moss IKS har deltatt i modellutprøvingen. Krisesentersekretariatet har sammen med Helsedirektoratet, Fylkesmannen i Østfolf, RVTS, KoRus og SAFO deltatt i referansegruppa som ble ledet av Bufdir.

Usynlighet, sårbarhet og avhengighet er tema som beskriver og forklarer vold mot personer med funksjonsnedsettelse. De som utsettes for vold har høyere alkoholkonsum enn kvinner som ikke utsettes for vold, og i tillegg til å ha rusproblemer har de også psykiske lidelser.

I krisesenterloven står det at det kommunale krisesentertilbudet skal omfatte et krisesenter eller tilsvarende botilbud. Tilgjengeligheten på krisesentertilbudet er avgjørende for om voldsutsatte oppsøker hjelp. Dersom det er begrensninger på tilbudssiden så begrenses tilfanget av personer som oppsøker krisesentrene. Voldsutsatte med tilleggsproblematikk passer ikke inn i de rammene som dagens krisesentre tilbyr, og reservasjonen for disse kvinnene for å søke hjelp er større enn for andre kvinner. Dette kommer blant annet av en oppfatning om at hjelpen ikke finnes.

Det er utviklet tre modeller som kan brukes for tilrettelegging av krisesentertjenester. Modellene er tilpasset de forskjellige gruppene og funnene viser at det å velge en av disse – og holde seg til den, gir best resultat. Modellene må benyttes som retningsgivende verktøy snarere enn oppskrifter på tilrettelegging for voldsutsatte med tilleggsproblematikk. Under alle modellene vil samarbeid om behandling i primær- og eller spesialisthelsetjeneste være nødvendig.

Flere former for tilrettelegging brukes ikke i dag, men kan med fordel benyttes. Et tilfredsstillende tilbud vil kreve en minstestandard av tilrettelegging. Et av rapportenes sentrale budskap er at det kan være hensiktsmessig for kommunene i sin videre tilrettelegging for gruppene å tenke bredere enn dagens strukturer, og vurdere i hvilken grad det kan tilrettelegges annerledes for de ulike brukergruppene. Nøkkelen er å finne en bevisst og hensiktsmessig balansegang mellom modellelementene.

Kommunene er særskilt forpliktet i henhold til lov om å gi voldsutsatte med tilleggsproblematikk et helhetlig krisesentertilbud.  På den annen side gir lovverket kommunen muligheten til å vurdere kravene til individuell tilpasning i hvert enkelt tilfelle. Dette gir kommunen rom for å nedprioritere et differensiert tilbud til disse gruppene til fordel for andre grupper.
